POLICIES

  • All clients must fill out a medical intake form/waiver to be reviewed at the time of appointment with the technician before the treatment will be completed

  • Clients under 18 must have medical intake form/waiver filled out by a legal guardian

  • Medical intake form/waiver will be sent in advance via email, please complete this form no less than 24 hours prior to your appointment to allow technician time to review

  • Occasionally I may have an appointment run long and you may be required to wait. Please be assured that I will give you the same level of care once we begin

  • Please arrive at your appointment no more than 5 minutes early and come to the front door at your scheduled time. You may park in the driveway

  • I will try to be as respectful of your time as possible, please be respectful of mine

  • Rescheduled appointments will only be allowed with more than 24 hours notice

  • If you cancel your appointment with less than 24 hours notice, are 15+ minutes late, or no-show at your scheduled time, you will be responsible for paying 50% of the full amount of the service booked before you will be able to reschedule 

  • If you prepaid for your service online, refunds will only be given with 24 hours or more notice of cancellation

  • Please come to your appointment alone as we do not have a seating area for spouses, children, friends etc..
